Comparing online and face-to-face student counselling: what therapeutic goals are identified and what are the implications for educational providers?

    Get PDF
Online counselling is increasingly being used as an alternative to face-to-face student counselling. Using an exploratory mixed methods design, this project investigates the practice by examining the types of therapeutic goals that 11 to 25 year olds identify online in routine practice. These goals are then compared to goals identified in equivalent school and community-based counselling services. 1,137 online goals (expressed by 504 young people) and 221 face-to-face goals (expressed by 220 young people) were analysed for key themes using grounded theory techniques. This analysis identified three core categories (1) Intrapersonal Goals, (2) Interpersonal Goals, and (3) Intrapersonal Goals directly related to others. Further statistical analysis of these themes indicated that online and face-to-face services appear to be being used in different ways by students. These differences are discussed alongside the implications for professionals working in educational settings

This is the author accepted manuscript. The final version is available from Wiley via the DOI in this record Assessment measures have been widely adopted into mental health settings incorporating idiographic (personalised) and nomothetic (standardised) approaches. Online mental health support services have developed significantly, but with very little testing of these measures. This study assesses the relative merits of acceptability, applicability and appropriateness of assessment measures online for children and young people. Acceptability was determined by whether users elected to complete the measure, applicability by whether they understood the questions, and appropriateness by their experience of completing the measure. Three measures, the Short Warwick‐Edinburgh Emotional and Mental Well‐Being Scale (SWEMWBS), the Young Person's Clinical Outcomes in Routine Evaluation (YP‐CORE) and the Strengths and Difficulties Questionnaire (SDQ), were randomly offered to 7,235 new registrations to assess relative acceptability of the measures, and two follow‐up questions were asked to assess applicability and appropriateness. The chi‐square tests of independence were calculated to compare response rates for each measure, followed by post hoc pairwise comparisons and effect size calculations. There was a high level of acceptability of the measures, with statistically significant differences between the less‐accepted SDQ and more‐accepted SWEMWBS. For applicability, the SWEMWBS was less understood and relatable than the other assessment measures. All the measures demonstrated a tolerable appropriateness. All differences were indicated at a small effect size. These findings support the acceptability, applicability and appropriateness of the use of assessment measures in an online setting. The SWEMWBS demonstrated surprisingly low applicability against the more clinical measures, suggesting further investigation into how young people ascribe meaning to the measures, and their motivations for completing them

Health promoting potential of herbal teas and tinctures from Artemisia campestris subsp maritima: from traditional remedies to prospective products

This work explored the biotechnological potential of the medicinal halophyte Artemisia campestris subsp. maritima (dune wormwood) as a source of health promoting commodities. For that purpose, infusions, decoctions and tinctures were prepared from roots and aerial-organs and evaluated for in vitro antioxidant, anti-diabetic and tyrosinase-inhibitory potential, and also for polyphenolic and mineral contents and toxicity. The dune wormwood extracts had high polyphenolic content and several phenolics were identified by ultra-high performance liquid chromatography-photodiode array-mass-spectrometry (UHPLC-PDA-MS). The main compounds were quinic, chlorogenic and caffeic acids, coumarin sulfates and dicaffeoylquinic acids; several of the identified phytoconstituents are here firstly reported in this A. campestris subspecies. Results obtained with this plant's extracts point to nutritional applications as mineral supplementary source, safe for human consumption, as suggested by the moderate to low toxicity of the extracts towards mammalian cell lines. The dune wormwood extracts had in general high antioxidant activity and also the capacity to inhibit a-glucosidase and tyrosinase. In summary, dune wormwood extracts are a significant source of polyphenolic and mineral constituents, antioxidants and a-glucosidase and tyrosinase inhibitors, and thus, relevant for different commercial segments like the pharmaceutical, cosmetic and/or food industries.FCT - Foundation for Science and Technology [CCMAR/Multi/04326/2013]; Portuguese National Budget; FCT [IF/00049/2012, SFRH/BD/94407/2013]; Research Foundation - Flanders (FWO) [12M8315N]info:eu-repo/semantics/publishedVersio

The Surgical Infection Society revised guidelines on the management of intra-abdominal infection

Background: Previous evidence-based guidelines on the management of intra-abdominal infection (IAI) were published by the Surgical Infection Society (SIS) in 1992, 2002, and 2010. At the time the most recent guideline was released, the plan was to update the guideline every five years to ensure the timeliness and appropriateness of the recommendations. Methods: Based on the previous guidelines, the task force outlined a number of topics related to the treatment of patients with IAI and then developed key questions on these various topics. All questions were approached using general and specific literature searches, focusing on articles and other information published since 2008. These publications and additional materials published before 2008 were reviewed by the task force as a whole or by individual subgroups as to relevance to individual questions. Recommendations were developed by a process of iterative consensus, with all task force members voting to accept or reject each recommendation. Grading was based on the GRADE (Grades of Recommendation Assessment, Development, and Evaluation) system; the quality of the evidence was graded as high, moderate, or weak, and the strength of the recommendation was graded as strong or weak. Review of the document was performed by members of the SIS who were not on the task force. After responses were made to all critiques, the document was approved as an official guideline of the SIS by the Executive Council. Results: This guideline summarizes the current recommendations developed by the task force on the treatment of patients who have IAI. Evidence-based recommendations have been made regarding risk assessment in individual patients; source control; the timing, selection, and duration of antimicrobial therapy; and suggested approaches to patients who fail initial therapy. Additional recommendations related to the treatment of pediatric patients with IAI have been included. Summary: The current recommendations of the SIS regarding the treatment of patients with IAI are provided in this guideline

Air quality and error quantity: pollution and performance in a high-skilled, quality-focused occupation

    Get PDF
We provide the first evidence that short-term exposure to air pollution affects the work performance of a group of highly-skilled, quality-focused employees. We repeatedly observe the decision-making of individual professional baseball umpires, quasi-randomly assigned to varying air quality across time and space. Unique characteristics of this setting combined with high-frequency data disentangle effects of multiple pollutants and identify previously under-explored acute effects. We find a 1 ppm increase in 3-hour CO causes an 11.5% increase in the propensity of umpires to make incorrect calls and a 10 mg/m3 increase in 12-hour PM2.5 causes a 2.6% increase. We control carefully for a variety of potential confounders and results are supported by robustness and falsification checks

Children and young people’s goals in online counselling: who achieves what, and how can the data impact on practice?

    No full text
Aim/Purpose: There is a growing awareness of the mental health difficulties that children and young people (CYP) encounter. Given the acknowledgement that large numbers of CYP will benefit from additional support, there is increased emphasis on creating accessible services. One area of growth in the UK has been the provision of online therapeutic services. Further, in attempting to make CYP services more youth friendly, goal-based outcome measures are commonly integrated into therapeutic work. This study investigates the types of therapeutic goals set by CYP online and considers how they develop. Design/Methodology: It makes use of a year of practice-based data generated from April 2016 to March 2017 on the Kooth.com service – a web-based platform that offers synchronous and asynchronous counselling, peer-to-peer support and self-help tools. 3403 goals were reported during this period by 1425 CYP and scored from 0-10. The demographic information of the clients using the online service is reflected upon before this information is considered alongside the change that is reported in goal scores. Ethical Approval: This study received a favourable ethical review at the institution of the third author. Results/Findings: It is notable that the online service was primarily used by female users during the reporting period (71%), and a majority of the service users were aged 13-16 years (62%). 46.5% of these goals were fully achieved by the cohort, with an average goal movement overall of 6.69. Conclusions/Implications: The findings contribute to the body of literature that reflect a similar demographic of CYP access online counselling as they do face-to-face counselling. When considering the level achievement of the commonest therapeutic goals, online services can provide an important role in aiding CYP in accessing additional support. Such a view highlights the importance of online services working in conjunction with face-to-face services
